Jason Mosley, 28, was working Tuesday at Hendrickson Equipment’s Smith Branch No. 1 operation in Knott County when material from the highwall above where he was drilling fell and crushed the cab of the unit he was working from, according to Associated Press.
Mosley, on the job as a drill operator for 10 months, was the third worker to die at a Kentucky surface operation this year. His body was recovered from the area at approximately 11am local time, according to the report, and the accident was reported to officials just before 10am.
According to the AP report, state Office of Mine Safety and Licensing representative Chuck Wolfe said the operation was cited twice for faulty equipment and once for inadequate emergency supplies since December of last year. Additionally, the mine had been inspected on two separate occasions since commencing operations in October 2005.
The Mine Safety and Health Administration’s records are slightly different, with 10 citations logged for equipment violations and hazardous material accumulations, the report went on to say.
The mine is owned by Leonard Hendrickson, a 30-year industry veteran.
