Mine worker Nigel James received second and third degree burns at BMA's Broadmeadow mine in February 2006, when the water truck he was driving rolled down a surface access ramp, collided with a conveyor and caught fire.
BMA Coal Operations, Moranbah-based contractor CQE Hire and mechanic Terence Atkins all pleaded guilty to breaches of the Coal Mining Safety and Health Act 1999, after investigations showed faults and poor maintenance were core factors in the incident.
Last year BMA was fined $A35,000 and ordered to pay costs of $9000.
This week, CQE was fined $44,000, and will pay $18,000 in investigation costs, while Atkins incurred a $4400 fine.
CQE and Atkins were each ordered to pay court costs of $66.50.
Altogether, penalties imposed on the three parties totalled $83,400 while total investigation costs reached $27,000.
