The operator was reversing at an angle into a corner to tip the load when the right side rear wheels pushed through a windrow, causing the truck to bottom out on the windrow.

The NSW Resources Regulator said the driver safely exited the truck.

"Mine operators should review their tipping procedures and consider controls such as tipping short in corners, using a spotter in similar circumstances and training, competency and experience of operators," it said.
