While the dump truck operator was attempting to reverse the truck to unload he noted the presence of the dozer, which was parked up next to the windrow waiting for the truck to dump.

However, he lost sight of the dozer during the manoeuvre and ended up reversing into it with one of truck wheels hitting the dozer's ripper box.

The regulator said operators must stop and reassess if they lose vision of other plant and equipment while reversing, as proceeding could put other workers at risk.
