Motorman Lee Edward Graham, 64, was fatally injured May 22 at Cleveland-Cliffs' Oak Grove mine when he was pinned between his motor and a longwall shearer body being transported underground.
According to the agency's preliminary report, 25-year mining veteran Graham, three other operators and one foreman were taking the shearer to the section when the carrier derailed while moving upgrade.
“The victim got out of his locomotive to assist in re-railing the car. He was between the locomotive and the carrier when the locomotive rolled or slid back, crushing him between the shearer body and locomotive," said MSHA.
To prevent future similar accidents at mines across the US, the agency issued the following best practices:
Never place yourself between rail cars/equipment unless positive stop blocks are in place;
Always set parking brakes when getting off mobile track equipment;
Pull, do not push rail cars; and
Always be aware of your location in relation to mobile equipment.
As always, the agency invites the input of the industry regarding other ways to work safely around moving equipment. Those wishing to submit suggestions should include the year and fatality number for the death.
Graham's death was the tenth in the nation so far this year, and the third classified by MSHA under Powered Haulage.
