Detailed in a mines inspectorate safety alert, the worker was attempting to break a line of auxiliary ventilation tubes.
The fan was still running with the consequential suction pressure making it a test of strength.
He eventually split the tube sections he was trying to separate, but the sudden jolt from the moving tubes caused the worker to fall out of the bucket and hit his head on the floor.
The inspectorate’s first point was that the training documents for the LHD prohibited working from the bucket.
A man basket, which could be attached to the LHD, was also nearby.
The inspectorate added that the hazard of the fan pressure acting on the tubes was not identified.
“There is clearly a need to follow agreed safe working procedures, and provide for effective supervision,” the inspectorate said.
Mines in the state were told to review the availability and location of work platforms in relation working areas, and to consider behavioural safety programs for their staff.
