That is one of the things the Department of Mines, Industry Regulation and Safety has determined from its investigation into the incident that claimed the life of David John Keen.
The department said it was working with WA Police and Rio Tinto to determine the cause of the incident.
However, “investigations so far indicate that the deceased acted independently and was not conducting work activities at the time”.
The department’s investigation is continuing.
WA Police has concluded its investigation and told Australia’s Mining Monthly it would be making no further comment while it prepared a report for the coroner.
Yandicoogina has returned to full operations.
Operations had been suspended immediately after the 51 year old from Warnbro’s death.
